We have many years of experience developing ISP’s for people with Spinal Cord and Acquired Brain Injuries, along with other complex care requirements and have successfully transitioned numerous ventilated quadriplegics into their local and rural homes from both hospital and residential settings. At Home Care is a preferred provider for the state Ventilator Dependent Quadriplegic Community Care Program (VDQ-CCP).
Specialised Nursing Care Plans are recorded in conjunction with the client, ensuring their individual goals are considered. These nursing plans are reviewed as indicated with noted changes to client’s capabilities.
At Home Care’s Clinical Nurse Managers' meet with each person and advocate and develop an Individualised Service Plan (ISP) detailing the transition to the community and their unique care requirements.
When assessing the requirements of an ISP we also consider the following factors for each person:
These factors play an important role in coordinating a persons care and ensure personal circumstances do not hinder them in achieving their goals.
Our in-house teams can incorporate registered nurses, enrolled nurses and support workers along with the availability to utilise disability support officers and students enrolled in allied health disciplines, such as physiotherapy, occupational therapy and speech pathology. Whilst core support is often provided by support workers, the teams all have access to clinical nurse managers to seek advice or further training to ensure their skills are current and relevant to each client's needs. This model allows us to care for people within the scope of their allocated funding.
Each client is managed under a Care Team Coordinator (CTC). The CTC's role is to ensure their teams of highly skilled staff are rostered to meet all the requirements in each specialised nursing care plan. This enables the delivery of high quality care to the community and provides the best opportunity for client's to achieve their personal goals.
This is a 24/7 service to ensure people are cared for whenever is required.
