We have many years of experience developing Individual Support Plans for people with Spinal Cord and Acquired Brain Injuries, and those with other complex care requirements.
At Home Care has successfully transitioned numerous ventilated quadriplegics into their local and rural homes from both hospital and residential settings. We care for a number of clients who require 24 hours of care per day and we have never failed to fill a shift.
This enables family and friends to be just that - family!
At Home Care is a preferred provider for the WA State Ventilator Dependent Quadriplegic Community Care Program (VDQ-CCP).
Specialised Nursing Care Plans are developed in conjunction with you, ensuring your individual goals are considered. These nursing plans are reviewed regularly as you capabilities change.
At Home Care’s Clinical Nurse Managers' meet with you and advocate and develop an Individualised Service Plan (ISP) detailing the transition to the community and your unique care requirements.
This includes:
When assessing the requirements of an ISP we also consider the following factors for each person:
These factors play an important role in coordinating your care and ensure personal circumstances do not hinder you in achieving your goals.
Our in-house teams incorporate Registered Nurses, Enrolled Nurses and Support Workers along with the availability to utilise Disability Support Officers and students enrolled in allied health disciplines, such as physiotherapy, occupational therapy and speech pathology. Whilst core support is often provided by support workers, the teams all have access to our Clinical Nurse Managers who offer advice and training to ensure their skills are current and relevant to each client's needs.
This model allows us to care for people with high care needs, within the scope of your allocated funding.
Each client is managed under a Care Team Coordinator (CTC). The CTC's role is to ensure their teams of highly skilled staff are rostered to meet all the requirements in each specialised nursing care plan. This enables the delivery of high quality care and provides the best opportunity for client's to achieve their personal goals.
This is a 24/7 service to ensure people are cared for whenever is required.
