We’ll work with you to understand your personal preferences and coordinate a team whose goal is to maximise your independence and provide you with the freedom to live your life how you choose.

NDIS Options

No matter which NDIS service model you decide to proceed with, you can always change your approach further down the track.

Full Agency

With a full agency service, At Home Care is responsible for the day to day management of your funding, staffing, training, supervision and compliance with NDIS requirements. You can relax with the peace of mind that your care is being managed by a professional and experienced team.

We recognise the importance of providing highly trained staff who listen and respond to both your needs and interests, and we encourage you to play an active part in choosing your carers - including the opportunity to meet potential candidates before agreeing to have them in your home.

Self-Management

Self-managing any part of your NDIS funding gives you increased choice and control over the services you receive.

Self-management means you are responsible for managing your NDIS funding independently. You still receive the quality care staff and services of At Home Care, but you manage the funds. You may also choose to manage other services such as rostering and interviewing.

Shared Management

You and your family may prefer to employ your own support staff, while using At Home Care+ to assist you with administration, insurances, staff payment and taxes, and backup home care staff (where required).
Shared management gives you the flexibility of recruiting your own staff with the peace of mind that comes with knowing the payment and administration is being handled by our professional staff.

What makes At Home Care a better choice?

Consultation with a NDIS Support Coordinator

We offer a consultation from an independent NDIS service consultant to make certain you maximise the support you receive.

NDIS Funding Tracker

Our fund tracker tool keeps you informed on the hours and dollars you have used against the hours and dollars still available in your NDIS plan. This gives you more control to ensure you are maximising your approved funding.

Personalised Nursing Care Plan (NCP)

For those with high care or medical needs our Clinical Nurse Manager will work with you to develop a custom Nursing Care Plan (NCP).

Meet and Greets and Buddy Shifts

As registered NDIS service providers, we understand how important it is to have a good relationship with the people looking after you. We give you the opportunity to meet your care team individually and take advantage of “buddy shifts”. This ensures you feel comfortable with your staff and that they are familiar with your needs and routine before providing care.

In-House Training

Prior to employment with At Home Care, all members of our team attend a compulsory in-house skills assessment and appropriate training. This ensures every team member meets our high standards in hoisting, medication administration and other high care requirements.

As Perth’s most trusted registered NDIS service provider, our goal is to  find a solution that works for you. For more information, please contact us on 08 9381 3344 or to discuss your care options.
