Where you have a patient that requires ongoing medical monitoring at home or in the workplace, our team ensures the data being received from the patient is properly managed and interpreted by experienced health professionals. Our care coordination team works in collaboration with the monitoring team to provide a scalable level of response, and escalation of care (as needed), for medically monitored patients as determined by their care plans.

Frequently asked questions

“What if I have any complications?”

You’ll have 24/7 access to medical professionals and our experienced nurses can provide advice on the majority of issues. Our staff also have a direct line to both your treating specialist and your GP for added peace of mind. If for any reason re-admission to hospital is required we will arrange all aspects of your transition.

“How long can I receive these services?”

Typically you will receive At Home Hospital services for the same period as you would be provided for your recovery / rehabilitation in hospital. For example, following an orthopaedic procedure you may be expected to remain in hospital for five days. If you discharge on day two you can then remain with At Home Hospital for a further three days. Your treating specialist will make the decision on when you can discharge. This may mean that you are offered care and nursing in your home for additional days.

“What treatments qualify?”

At Home Hospital is available for all rehabilitation or recovery periods typically requiring a hospital stay. These range from recovery from orthopaedic surgery to maternity.
